A method includes wirelessly receiving quality-of-care information from a patient mobile communication device, reporting the quality-of-care information to a quality tracking system, and receiving an incentive payment responsive to reporting the quality-of-care information to the quality tracking system. The quality-of-care information may relate to one or more quality-of-care indicators used to incentivize a provider or consumer of healthcare services. A patient may be alerted to the quality-of-care indicators for specific medical conditions. Personalized and specific notifications generated by an application residing in the patient's mobile device or running in the cloud may be communicated to the patient electronically, including prompts to the patient to take specific action in order to best manage a medical condition or prescribed treatment. A healthcare provider may be notified of a treatment or quality-of-care recommended action to be taken.
