Superior Labrum Anterior to Posterior (SLAP) 병변과 관절 와 상완 관절 불안정성

견관절 안정성에 있어서 관절 와 순 상완 이두 근 장두 복합체의 역할은 명확하지는 않지만 젊은 환자, 특히 운동 선수의 경우 superior labrum anterior to posterior (SLAP) 단독 병변만으로 불안정성이 야기될 수 있고, 급성 혹은 만성 견관절의 탈구에서 SLAP 병변은 흔한 동반 병리 중 하나인 것은 잘 알려진 사실이다. SLAP 병변은 X형까지 분류되어 있는데 II형, 그리고 IV형부터 X형까지가 관절 불안정성과 관련이 있다. 관절 불안정성이 동반된 젊은 환자에서 관절경 수술이 선호되는 진단 및 치료 수단이며, 수술 시 관절 안정성에 필요한 관절 와 순의 모든 해부학적 문제를 복구하고자 노력해야 한다. 관절 안정성 회복의 측면에서 SLAP 및 Bankart 병변에 대한 관절경적 봉합술의 좋은 임상적 결과들을 보고하고 있으나 장기적인 경과 관찰이 더 필요하고, SLAP 병변과 관절 와 상완 관절 불안정성 간의 관계에 대한 연구가 더 요구된다. Role of the superior labrum-biceps complex for the glenohumeral stability is still unclear. Nevertheless, isolated superior labrum anterior to posterior (SLAP) lesion can cause glenohumeral instability in young patient (especially in throwing athletes) and SLAP lesion are a well-known pathology entity in acute or chronic glenohumeral dislocation. Ten types of SLAP lesion have been classified by arthroscopic examination, among them type II and type IV through X SLAP can disturb glenohumeral stability by disrupting the anchoring of biceps. Arthroscopic repair of labrum is the most preferred method for SLAP lesion with glenohumeral instability in younger patient. Surgical treatment, if necessary, should address all aspects of the labral anatomy so that all the roles of the labrum in shoulder stability must be restored. In terms of restoration for glenohumeral instability, the good clinical results have been reported after arthroscopic repair of a SLAP with Bankart lesion. But, it is still a work in progress for long term clinical follow-up and understanding about relationship between SLAP lesion and glenohumeral instability. KCI Citation Count: 0
