비전형적인 위치에서 발생한 누선 선상 낭포암 1예

누선의 선상 낭포암(adenoid cystic carcinoma)은 누선의 악성 종양 중 가장 흔한 빈도를 보이나 실제 임상에서는 매우 드문 질환으로 여러 가지 치료법을 실시하더라도 2년 생존율이 50% 이하인 예후가 대단히 불량한 악성종양이다. 저자들은 62세 여자 환자에서 악성 종양에 대한 다른 증상은 나타나지 않고 우안의 경도의 안구돌출만을 호소하는 것을 관찰하였다. 안와 자기공명영상(MRI)상 일반적인 누선의 선상 낭포암의 위치와 다르게 우측 안와의 상이측으로 경계가 비교적 명확하고, 시신경을 비측으로 밀고 있는 소견을 보였으나, 주위 신경조직이나 골조직으로의 침윤이나 전이소견이 없는 종물이 관찰되어 종물 절제술만을 시행하였다. 조직검사상 누선의 선상 낭포암으로 진단 받고 경과관찰 중인 1예에서 병리조직검사상 다양한 형태를 보였기에 문헌고찰과 함께 보고하는 바이다.(한안지 39:2859-2863, 1998) An adenoid cystic carcinoma of the lacrimal gland is the most common epithelial neoplasms in lacrimal gland. But it is rare in clinical practice and is poor prognosis below 50% at 2 year survival rate with various clinical management. We experienced a case of a 62-year-old woman who had complained just proptosis and no other tumor symtoms. Being different from the other adenoid cystic carcinoma of the lacrimal gland, MRI revealed mass of the right superiotemporal lacrimal gland which was well demarcated and displaced in nasal area to the optic nerve and no metastasis to adjacent nerves and bones. We performed the lateral orbitotomy followed by macroscopical complete resection of tumors. The histopathologic findings revealed the adenoid cystic carcinoma of the lacrimal gland and were showed variable. So we would report our case with a brief review of the literature(J Korean Ophthalmol Soc 39:2859-2863, 1998)
