내측 상하 눈썹에 의한 각막 자극이 있는 환자에서 상안검 및 하안검 성형술의 효과

목적 : 동양 어린이에게 흔히 발견되는 선천적 안검 기형인 내측눈구석주름이나 덧눈꺼풀은 미용적인 문제 외에도 상하 내측 속눈썹에 의해 각막 자극이 발생하여 수술적 교정이 필요한 경우가 있다. 내측눈구석주름을 교정하는 방법으로는 Y-V plasty나 Mustarde double Z-plasty가 일반적이나 미용상 반흔이 문제되어 많이 시도되지는 않는다. 대상과 방법 : 저자들은 상하 내측 속눈썹에 의해 각막 자극을 보이는 환아 26명 52안(여자 19명 38안, 남자 7명 14안)을 대상으로 상안검 및 하안검성형술을 동시에 시행하여 그 결과 및 합병증을 의무기록을 통한 후향적 조사방법으로 관찰하였다. 결과 : epicanthus tarsalis와 덧눈꺼풀이 동반된 환아는 22명 44안이었고 epicanthus palpebralis 환아는 4명 8안이었다. 수술 당시 나이는 3.7세에서 10.3세로 평균 7.0세이었다. 술후 6개월 이상 경과관찰 결과 쌍커풀 비대칭 1명, 쌍커풀 풀림 1명 2안, 하안검 저교정 2명 3안이 나타났다. 결론 : 상하 내측 속눈썹에 의해 각막 자극을 보이는 환아를 위한 상안검 및 하안검 동시 성형술은 기존의 수술법에 비해 미용적인 해결은 부족하지만 반흔이 보이지 않는 장점이 있고 각막자극 증상을 완화시키는 좋은 방법으로 생각된다. Purpose : Epicanthal folds and epiblepharon which often develop in oriental children are congenital anomalies of eyelids. These may need surgical correction because corneal irritation may develop by medial upper and lower eyelash besides cosmetic problem V-Y plasy and Mustarde`s method are standards for correcton of epicanthal folds, but not usually performed because of cosmetic scar formation. Methods : This study included 26 patients, 52 eyes(19 females, 38 eyes; 7 males 14 eyes) with corneal irritation due to medial upper and lower eyelash. We performed upper and lower lid blepharoplasy simultaneously and analyzed their results and complications through medical records. Results : Epiblepharon associated with epicnathus tarsalis was found in 22 patients, 22 eyes, epicanthus palpebralis in 4 patients, 8 eyes. At the time of surgery mean age was 7 years old (3.7~10.3 years old). During the follow-up more than 6 months, double fold asymmetry was found in 1 patient, double fold loosening in 1 patient, 2 eyes, lower eyelid undercorrection in 2 patients, 3 eyes. Conclusions : These findings suggest that simultaneous upper and lower lid blepharoplasty for patients with corneal problem due to medial upper and lower eyelash is more unsatisfactory in the cosmetie result than standard method, but has advantage of less scarring, and thought to be a good method to relieve corneal irritation.
