각막이식과 경면현미경을 통한 내피세포의 변화

전체층 각막이식후 1주, 1개월, 3개월, 6개월에 각막내피세포의 변화를 연속적으로 측정하여 수여자의 나이, 기증자의 나이, 기증자의 술전 각막 내피세포수, 수여자의 술전 진단명과 내피세포 소실사이의 상관관계를 알아보고자 하였다. 1996년 12월부터 1999년 1월까지 전북대학교 병원에서 각막이식 수술을 받은 70명의 환자중 6개월 이상 비접촉 경면현미경으로 내피세포 촬영이 가능했던 30안을 대상으로 결과를 분석하였다. 관찰기간중 계속적인 세포소실이 있었으며 술후 1주, 1개월, 3개월, 6개월의 세포소실율은 각각 11.5%, 19.0%, 25.0%, 30.0%이었다. 각 시기의 세포소실율은 기증자의 나이, 수여자의 나이, 술전 내피 세포수와는 통계학적으로 유의한 상관 관계는 없었다(r＜±0.4, p＞0.05). 각 시기별 술전 내피세포수와 술후 내피세포수와는 유의한 상관관계가 있었다(r＞0.7, p＜0.05)). 술후 3개월에서 원추각막군이 수포성 각막병증군이나 각막 백반군보다 세포소실율이 유의하게 낮았다(Wilcoxon test, p＜0.05) (한안지 41:1124~1131, 2000). The purpose of this study was to investigate the correlations between endothelial cell loss rate at one week, one month, three months, and six months after penetrating keratoplasty and clinical factors including recipient age, donor age, preoperative endothelial cell densities of the donors and preoperative diagnoses of the recipients. Among the 70 patients who underwent penetrating keratoplasty at Chonbuk University Hospital from December 1996 to January 1999, 30 eyes of 30 patients that showed reasonable endothelial resolution by non-contact specular microscopy during the follow up period of 6 months were chosen and evaluated. The cell density decreased continuously during the observation period. Endothelial cell loss rate averaged 11.5% at one week, 19.0% at one month, 25.0% at three months and 30.0%at six months after penetrating keratoplasty. Endothelial cell loss rate showed no significant correlation with donor age, recipient age, preoperative cell density at any examination period(r＜±0.4,p＞0.05). Preoperative endothelial cell density showed significant correlation with postoperative endothelial cell density at each examination period(r＞0.7, p＜0.05). The rate of endothelial loss in the keratoconus group was significantly lower than those of bullous keratopathy or corneal leukoma groups at three months after penetrating Keratoplasty(Wilcoxon test,p＜0.05) (J Korean Ophthalmol Soc 41:1124~1131, 2000).
