녹내장 환자에서 접촉적 Diode 레이저 경공막 광응고술의 효과

약물 및 수술요법으로 안압조절이 되지 않는 말기 녹내장 환자에게 810mm의 파장을 가진 Diode레이저로 공막을 통한 접촉적 모양체 광응고술을 시행한 후 안압의 변화 및 부작용을 관찰하여,Diode 레이저 조사시 안전한 에너지양과 적정조사 범위를 알아보고자 하였다. Diode레이저를 말기 녹내장 환자(15명 15안)에 각각 3J로 조사한 A군(6명 6안)과 4J로 조사한 B군(9명 9안)으로 나누어 접촉적 경공막 광응고술을 시행한 후 1일,1주,1개월,2개월,3개월,4개월,5개월,6개월에 각각 안압을 측정하였다. A군과 B군의 최종 안압 감소율은 A군이 83.02%를 B군이 64.72%의 최종 안압 감소율을 보였고,A군의 성공률은 50%,B군의 성공률은 66.7%로 나타났다. Diode레이저를 이용하여 안압하강 효과를 관찰하였으며 이와같은 결과를 응용함으로써 조절이 되지 않는 말기 녹내장 환자의 치료에 많은 도움이 될것으로 생각된다(한안지 38:659~665, 1997). We observed the changes of IOP after contact transscleral cyclophoto-coagualtion with Diode laser which emit 810mm beam and we tride to investigate the suitable level and range of energy when Diode laser was applied to the refractory glaucoma patients. Fifteen eyes underwent contact transscleral cyclophotocoagulation with energy 3J(Group A, 6eyes), 4J(Group B, 9eyes) separately and intraocular pressure(IOP) was measured at 1 day, 1 week, 1 month, 2 month, 3 month, 4month, 5month, 6month, postoperatively. The intraocular pressure(IOP) decreasing rate was 83.02% in A group and 64.72% in B group. The success rate was 50% in A group and 66.7% in B group at 6 month postoperatively. It is suggested that contact transscleral cyclophotocoagulation with Diode laser si useful in lowering IOP in refractory glaucoma patients(J Korean Ophthalmol Soc 38:659~665, 1997).
