Abstract | Purpose: To evaluate the exact prevalence of primary glomerular diseases in Korea. Methods: We analyzed a retrospective cohort of biopsy proven 100 patients with primary glomerular disease in OO Hospital from April 1990 to March 2010. Results: Pathologic diagnosises of 100 cases were as follows: IgA nephropathy (IgAN) , 557 cases (50.6%), was the most common followed by 200 cases (18.1%) of minor glomerular abnormalities (MGA), 168 cases (15.2%) of focal segmental glomerulosclerosis (FSGS), 93 cases (8.0%) of membranous nephropathy (MN), 31 cases (2.8%) of membranoproliferative glomerulonephritis type I (MPGN), 17 cases (1.5%) of focal glomerulonephritis and 7 cases (0.6%) of diffuse mesangial proliferative glomerulonephritis (DMGN) in order. In idiopathic nephrotic syndrome, the most common pathologic diagnosis was minimal change nephrotic syndrome (MCNS) (40.2%), followed by FSGS (27.5%), MN (24.2%), MPGN (8.1%) and DMGN (0.5%). When the incidence rates between 1990-1992 and 2008-2010 were compared, IgAN and FSGS increased from 34.12.5 to 47.8%, 30.4%, but MCNS (from 33.3 to 6.5 %) decreased significantly Conclusion: IgAN was the most common primary glomerulonephritis. During the past 20 years, the prevalence of IgAN and FSGS were increased, while MCNS and MN were decreased. |

During the past 20 years, the prevalence of IgAN and FSGS were increased, while MCNS and MN were decreased. 목적: 일차성 사구체질환과 신증후군의 발생 빈도를 알기 위해서 본 연구를 시행하였다. 방 법: 1990년 4월부터 2010년 3월까지 만 20년 동안 동아대학교병원 내과에서 검사실 검사 및 신생검에 의해 일차성 사구체 신염으로 진단된 1,100예를 신생검을 위한 입원 시 소견을 바탕으로 후향적인 분석을 시행하였다. 결 과: 이 기간 중 총 1,633명의 환자에서 신생검을 시행하였으며 이 중 일차성 사구체신염은 1,100예 (67.4%) 이었다. 대상 예는 남자 613예 여자 487예로 남녀 비는 1.31:1, 평균연령은 36±15 (10-81)세 이었다. 병리학적 소견에 의한 빈도는 IgA신병증 (IgAN)이 557예 (50.6%), 미세변화 (MGA) 200예 (18.1%), 초점분절사구체경화증 (FSGS) 168예 (15.2 %), 막성사구체신염 (MN) 93예 (8%), 막증식성사구체신염 (MPGN) 31예 (2.8%)의 순이었다. 1990년부터 20년을 2년 단위로 10기로 나누어 조사한 연대별 발생 빈도는 1기와 10기가 IgAN은 34.7%에서 47.8%으로 증가, MGA는 36.1%에서 11.9%로 감소, FSGS는 12.5%에서 30.4%로 증가, MN 은 6.9%에서 3.2%로 감소하였다. 특발성 신증후군군은 385예 (35%)이었고 미세변화형신증후군 (MCNS) 155예 (40.2 %), FSGS 106예 (27.5%), MN 93예 (24.2%), MPGN- I 31예 (8.1%)였다. 연대 별로는 1기와 10기가 각각 MCNS 33.3%, 6.5%, FSGS 9.8%, 12.9%, MN 4.2%, 3.2%, MPGN 1.4%, 1.1%로 MCNS는 감소하였고 FSGS는 증가하였다. IgAN은 총 557예 중 51예 (9.1%)에서 신증후군을 동반하였으며 Hass 분류법에 의한 subclass II와 III가 각각 150예, 155예로 높은 빈도를 보였고 subclass I, IV가 각각 100예, 95예, subclass V도 41예 였다. 결론: 본 연구에서 IgAN은 가장 흔한 원발성 사구체신염의 질환이며 특발성 신증후군을 유발하는 가장 흔한 질환은 MCNS이다. 또한 최근 20년간 IgAN과 FSGS는 그 빈도가 증가, MCNS과 MN는 감소하는 추세이다. 향후 국내 신장질환발생의 빈도 및 시기에 따른 변화를 확인하기 위해서는 대규모 환자군을 대상으로 한 다기관 연구가 필요할 것으로 사료된다.
