Summary: | Purpose: This study aimed to investigate the influence of functional leg length discrepancy (LLD) on health-related quality of life after total hip arthroplasty (THA). Methods: We enrolled 42 patients who had undergone unilateral THA. Structural and functional LLDs were assessed 3 weeks after THA. Patient-perceived LLD and Short-Form 36-Item Health Survey version 2 (SF-36v2) were surveyed 6 months after THA. We performed univariate analyses of the relationships between each measurement in order to select items for the path model and calculated path coefficients and the fit of the model through path analysis. Results: The functional and patient-perceived LLDs, and the physical component summary (PCS) score of SF-36v2 were selected for the path model. The patient-perceived LLD was affected from functional LLD. The PCS score was affected from patient-perceived LLD. The fit indexes of the model showed adequate values. (Goodness-of-Fit Index = 0.974, Adjusted Goodness-of-Fit Index = 0.945, Root Mean Square Error of Approximation < 0.001). Conclusion: The study suggest that the interventions to improve the functional LLD are important for patients with patient-perceived LLD and poor health-related outcomes after THA.
【目的】本研究の目的は,人工股関節置換術(以下,THA)後の健康関連QOLに対して機能的脚長差が影響を及ぼすまでの一連の障害構造モデルについてパス解析を用い検討することである。【方法】THA 後患者42名を対象に,術後3週の構造的脚長差と機能的脚長差,術後6ヵ月後の主観的脚長差,SF-36のサマリースコアを評価した。モデルへ投入する項目を選択するために単変量解析を行い,その後にパス係数とモデルの適合度を求めるためにパス解析を行った。【結果】モデルには機能的脚長差,主観的脚長差,身体的コンポーネント・サマリースコア(以下,PCS)が選択された。パス解析の結果,機能的脚長差は主観的脚長差に影響し,主観的脚長差はPCSに影響していることが示された。このモデルは十分な適合度を示した。【結論】THA後主観的脚長差をもち身体的健康関連QOLが低下した患者に対して,機能的脚長差に対する治療的介入が重要であることが示唆された。 |
