Summary:Recently, the relationship between laryngeal granuloma and gastro esophageal reflux disease (GERD) has been investigated. We performed esophageal pH monitoring of laryngeal granuloma patients for 24 hours in order to diagnose GERD. The subjects consisted of laryngeal granuloma patients except for intubation glanulation patients who visited the Nagoya City University Hospital otorhinolaryngology out-patient clinic between January 2003 and May 2005. Esophageal pH monitoring was done in these 8 patients for 24 hours. Any patient whose time ratio under pH4 of the lower esophagus was 4% for more than 24 hours was diagnosed to have GERD. Six of 8 patients (75%) were diagnosed to have GERD. We administered proton pump inhibitor (PPI) and reflux esophagitis treatment medicine to those six patients for twelve weeks, and thereafter the effect was examined. The granuloma did not heal in 2 patients, while the laryngeal granuloma disappeared in 4 patients. The lower esophageal reflux time ratio of the patients whose granulation did not heal was 20% and more. Though PPI was effective for the treatment of laryngeal granuloma, the individual effect of PPI was found to be insufficient in very severe cases of GERD.
