ヒトにおける遅伝導路の解剖学的位置に関し解剖学的に2つ存在することを Inoue, Becker が報告し, それぞれ, right inferior extension, left inferior extension と名付けた1). オクラホマ大学の Jackman は, multiple forms AV nodal reentrant tachycardia の成因を2つの房室結節(AVN)伝導路の間を旋回する頻拍であるが, 房室結節伝導路には解剖学的に5つ以上存在すると述べている2). すなわち, 1本の速伝導路(fast), 4本の遅伝導路{slow pathway (SP)}であり, 遅伝導路はその付着部位により, rightward inferior extension of the AVN, leftward inferior extension of the AVN, inferolateral left atrial slow pathway, さらに, Kaneko らにより報告された anterior superior slow pathway3), である. そのため, 9種類以上の房室結節回帰性頻拍(AVNRT)が存在し得る. すなわち, slow/fast AVNRT≧3種類, slow/slow AVNRT≧3種類, fast/slow AVNRT≧3種類のAVNRTである. Jackman は房室伝導時間ではなく, SPの解剖学的位置により分類している. 西村らの症例では, 3回の房室伝導の jump up を認めたため, それぞれ, intermediate pathway, slow pathway, very slow pathway と呼び, AVNの right inferior extension 部位での通電で, slow および very slow pathway が消失したと報告している. Slow および very slow pathway が right inferior extension 内での縦解離に起因しているのか, AVN right inferior extension と leftward inferior extension が一部重複しているのか興味が持たれる.
