多系統萎縮症における眼球運動障害―50例における経時的検討

多系統萎縮症における眼球運動障害の特徴を明らかにするため,50例を対象に9種類の眼運動系パラメーターの経時的変化を検討した.各パラメーターにおける異常出現率は,いずれも病型間(小脳失調先行型とパーキンソニズム先行型)で有意差をみとめなかった.また,経時的変化からパラメーターは3群―病初期から異常出現率が高頻度にみとめられる群,経過とともに徐々に高率化する群,進行しても比較的低値にとどまる群―に大別された.各群のそれぞれの代表として,頭位変換性眼振,視性抑制反応および温度眼振に注目し,これらの機能解剖学的検討から,病変は小脳の背側虫部次いで片葉,そして前庭神経核から前庭皮質にもおよぶと考えた....
