ST合剤の併用が有効と考えられたWegener肉芽腫症の1例

ST合剤とステロイドの併用が有効であったWegener肉芽腫症の1例を報告する。症例は62歳の女性。主訴は咳と血尿。胸部写真では, 左肺尖部に不正形の淡い肺炎陰影を認めた。肺炎は抗生剤にて軽快したが蛋白尿は1日2.5g認め, 血尿も改善せず, 血清クレアチニンも2.5mg/dlと上昇を認めた。C-ANCAは58EIA unitsと陽性で, 腎生検では糸球体の67%に細胞性半月体形成を認め, 蛍光抗体法ではIgA, IgG, C3の沈着は認めなかった。以上よりWegener肉芽腫症と診断した。ステロイドとST合剤の併用にてC-ANCAは陰性化し蛋白尿と血尿は消失し血清クレアチニンも正常化した。S...
