Experience of Liver Transplant for Unresectable Colorectal Liver Metastases at a Single High-Volume Center in North America

Summary:Background: Increasing evidence has demonstrated the benefit of liver transplant (LT) for unresectable colorectal liver metastases (urCRLM). We report the retrospective review of patients with urCRLM who underwent LDLT from July 2019 to April 2023 at a single center in North America.Results: Among 138 patients, 112 (81%) were not candidates due to progression /extrahepatic disease, 12 (9%) are still candidates and 14 (10%) underwent LT. Before LT, the median number of chemotherapy cycles was 24, with a median time on chemotherapy of 1.3 years. The median time between the primary tumor resection and the LT was 7.3 months. The indication for LT was unresectability in 13 patients (93%) and liver failure due to oncological treatment in one patient. Three patients (21%) had Oslo score of 0 and 11 (79%) had 1. Five patients developed recurrence. One had at lungs and lymph nodes and died at 20 months. The other 4 are alive. Recurrence-free and overall survival at 18 months were 65% and 100% respectively.Conclusions: This single-center cohort in North America demonstrates that recurrence-free and overall survival after LT for urCRLM justifies LT as a treatment option for selected patients.
