Summary:Six cases of colorectal carcinoma smaller than 1cm in maximum diameter are presented. Mean age was 60.3 years. Sex ratio of male to female was 5:1. No clinical symptoms were seen except in one case, in which melena was observed. Four out of the six cases were found to have colon polyps, two cases revealed colon cancer and two cases gastric cancer. The precise diagnosis was done in only one case by barium enema examination. On the other hand, three cases of type IIa+IIc were correctly diagnosed by the endoscopical examination, whereas other three cases of protruding type carcinoma were diagnosed by histological examination after polypectomy. For the early and correct diagnosis, the endoscopical examination was more useful than barium enema. It would be acceptable that all the protruding lesions even smaller than 1cm must be polypectomysed. Colonoscopic findings of the lesions showed protruding type of the tumor in three cases and type IIa+IIc in other three cases. In a case of type IIa+IIc, the invasion to pm layer was found. The other one case revealed sm invasion. The authors consider that the two types of mode of the growth and progression may exist. Five out of the six cases had adenomatous components in the tumors. This means that adenoma is closely connected with colon cancer.
