Tumefactive multiple sclerosisの1例
Tumef active multiple sclerosis (tMS)は脱髄疾患であるが,画像所見では悪性脳腫瘍との鑑別が困難であり,組織診を要することが多い.左下肢の違和感を主訴に来院した37歳男性に,MRI上tMSが疑われた.髄液所見は正常であったが,症状が急速に悪化したため,3日間のステロイドパルス療法を施した.治療後のMRIでは病巣部のさらなる拡大を認め,tMSとしては非典型的な経過を呈したため,生検術を施行したところ,脱髄疾患と診断され,画像所見を合わせtMSの診断に至った.本疾患に対し早期に的確な組織診断を行うことは,脳神経外科医の重要な役割である....
