膵頭十二指腸切除術中の門脈遮断による重度腸管鬱血に対し12時間後の二期再建が有効であった1例
膵頭部癌に対する膵頭十二指腸切除術で,上腸間膜静脈,門脈の広範な損傷で長時間の門脈遮断を余儀なくされ,重度の腸管鬱血のため再建は二期的に行ったが,鬱血や浮腫は消退し良好に行いえた症例を経験したので報告する.症例は59歳の男性で,膵頭部癌(cT3N0M0,Stage IIA)に対し膵頭十二指腸切除術を計画した.膵臓と上腸間膜静脈,門脈周囲は非癌部でも広範に随伴性膵炎による癒着がみられた.剥離の際,複数箇所損傷を来し門脈のクランプを余儀なくされた.小腸に著明な鬱血・浮腫がみられ,アンスロンtubeによるバイパスを併施し門脈再建を行ったが改善せず,縫合不全のリスクを考慮し再建は二期的に行うこととした...

Abstract | 膵頭部癌に対する膵頭十二指腸切除術で,上腸間膜静脈,門脈の広範な損傷で長時間の門脈遮断を余儀なくされ,重度の腸管鬱血のため再建は二期的に行ったが,鬱血や浮腫は消退し良好に行いえた症例を経験したので報告する.症例は59歳の男性で,膵頭部癌(cT3N0M0,Stage IIA)に対し膵頭十二指腸切除術を計画した.膵臓と上腸間膜静脈,門脈周囲は非癌部でも広範に随伴性膵炎による癒着がみられた.剥離の際,複数箇所損傷を来し門脈のクランプを余儀なくされた.小腸に著明な鬱血・浮腫がみられ,アンスロンtubeによるバイパスを併施し門脈再建を行ったが改善せず,縫合不全のリスクを考慮し再建は二期的に行うこととした.再開腹は12時間後に行ったが腸管の鬱血・浮腫はほぼ消退しており,Child変法で再建し術後経過は良好であった.再建腸管に著明な鬱血や浮腫が見られる場合には,二期再建も有効な選択枝と考えられた. |
