胆囊癌孤立性膵転移の1例
症例は54歳の男性で，膵・胆管合流異常症に合併する胆囊癌に対して胆囊全層切除，肝外胆管切除，リンパ節郭清，胆管空腸吻合術を施行された．病理組織学的診断は中分化型腺癌，pT2，ly0，v1，pN0，pStage IIであった．術後2年10か月，腹部超音波検査にて膵体部に不整形で遷延性の造影効果を伴う11 mm大の低エコー腫瘤を認めた．超音波内視鏡下穿刺生検を施行し，腺癌の組織診断を得た．膵体部癌cT3N0M0 cStage IIAと診断し，尾側膵切除を施行した．病理組織診断では膵体部に2か所の病変を認め，いずれの病変も低分化から中分化型の腺癌の所見であった．HE染色および免疫染色検査での形態・形...

| Abstract | 症例は54歳の男性で，膵・胆管合流異常症に合併する胆囊癌に対して胆囊全層切除，肝外胆管切除，リンパ節郭清，胆管空腸吻合術を施行された．病理組織学的診断は中分化型腺癌，pT2，ly0，v1，pN0，pStage IIであった．術後2年10か月，腹部超音波検査にて膵体部に不整形で遷延性の造影効果を伴う11 mm大の低エコー腫瘤を認めた．超音波内視鏡下穿刺生検を施行し，腺癌の組織診断を得た．膵体部癌cT3N0M0 cStage IIAと診断し，尾側膵切除を施行した．病理組織診断では膵体部に2か所の病変を認め，いずれの病変も低分化から中分化型の腺癌の所見であった．HE染色および免疫染色検査での形態・形質が既往の胆囊癌の病理組織学的検査所見と一致していたため，胆囊癌膵転移と診断した．術後18か月現在，無再発生存中である．転移性膵腫瘍の多くが腎細胞癌，肉腫，大腸癌，悪性黒色腫からの転移であり，胆囊癌の膵転移は極めてまれである．胆囊癌の孤立性膵転移について報告する． |
