| Summary: | Elongated styloid process (ESP) is caused by calcification or ossification of the stylohyoid ligament. It is diagnosed when the length of the styloid process exceeds 30 mm and the patient presents with various cervicofacial symptoms. We report a case of ESP of about 70 mm that extended to vicinity of the hyoid bone over a period of several years, treated by cervical incision.  A 66-year-old female visited an otolaryngologist and psychosomatic department for tinnitus and neck pain on the left side that had persisted for several years. She was diagnosed with depression and was taking medication. She visited our department and was diagnosed with ESP in January 2017. Despite understanding the indication for surgery, the patient discontinued treatment voluntarily due to anxiety. She visited our department again with a desire to have surgery in August 2020. The styloid process was palpable just below the palatine tonsil from the oral cavity behind the left pterygomandibular raphe. 3D-CT showed more extension of the left styloid process into the hyoid bone than at the initial examination.  Therefore, we excised the left styloid process by cervical incision. We observed migration of fibrous connective tissue and bone-like tissue in pathological findings. She had no neck pain early after surgery and showed good progress. |
