Abstract In order to determine whether age and coronary risk factors influence standard 12 lead ECG variables, 8966 male and 1618 female NTT workers, aged 20 to 59 years who underwent health examination at NTT Tokyo health administration center from April to September in 1990, were studied. These workers were divided into two groups as follows : (1) General subjects consistng of 1112 males and 716 females aged 20 to 59 years who were selected randomly out of the 8966 males and 1618 females. (2) Control subjects consisting of 1019 males and 81 females aged 30 to 59 years without hypercholesteremia, diabetes melitis, obesity, hypertension, cigarette smoking, electroionic abnormalities and systemic disorder. In an earlier study (1989), we reported the significant prolongation of PQ interval, QRS duration and QTc width and QRS axis deviation to the left on frontal plane developed with age, but with no significant change in heart rate. In the present study similar results as a 1989 study were observed in general subjects. In control subjects, these significant changes of standard ECG variables except for QTc width in male were diminished in both sexes with age. These results indicate that coronary risk factors electroionic abnormalities and systemic disorders may be factors that determine changes in electrophysiological properties which were evaluated by standard 12 lead ECG seen in the process of aging.

These results indicate that coronary risk factors electroionic abnormalities and systemic disorders may be factors that determine changes in electrophysiological properties which were evaluated by standard 12 lead ECG seen in the process of aging. 本研究は加齢と冠危険因子が標準12誘導心電図パラメータに及ぼす影響を観察した。対象は1990年4月より9月までに健診を受けた20歳より59歳までの男性8966人と女性1618のうち無作為抽出した男性1112人と女性716人を一般社員として検討した。また, 30歳より59歳までの男性7980人と女性1274人から高コレステロール血症, 糖尿病, 肥満, 高血圧症, 喫煙などの冠危険因子, 電解質異常および器質的疾患などを除外した男性1019人と女性81人をコントロール集団とした。一般社員では加齢にともないPQ時間とQTcの延長, 前額面QRS軸の左軸偏位を男女ともに認めたが, 心拍数には加齢にともなう変化は男女とも認められなかった。さらに, ECGパラメータの性差は20歳代から50歳代の各年齢層で男性は女性に比し, PQ, QRS時間は延長し, QTcは短縮が認められた。すなわち, 1989年に報告した30歳より59歳までの12528人を対象にした研究と同様な結果が得られた。コントロール集団では加齢にともなう変化として, QTcの延長が男性に認められたのみであった。コントロール集団における心電図パラメータの性差は女性が81人と少なく加齢との関連について検討することができなかったが, 全年齢の平均でみると男性は女性に比しPQおよびQRS時間の延長が認められ, QTcは有意に短縮していた。すなわち, 一般社員における各年齢層で検討した性差と同様な結果であった。これらの結果は冠危険因子, 電解質異常, 器質的疾患が加齢にともなう心拍数, PQ時間, QRS時間, QTcや前額面QRS軸などの電気生理学的特性の変化を決定する要因となりうるものと推測された。
