| Abstract_FL | OBJECTIVE:To probe into the role of clinical pharmacists in individual medication treatment . METHODS:Retrospective analysis was conducted on the participation of clinical pharmacists in individual treatment of one patient with renal insufficiency after heart valve replacement . RESULTS: Clinical pharmacists implemented individualized anti-infection and anticoagulant therapy and pharmaceutical care for the patients by the application of individualized drug delivery technology in terms of therapeutic drug monitoring and gene detection , and synthetically consideration of the special pathological state of the patients and drug combination , which improved the clinical efficacy and promoted the rational drug use .CONCLUSIONS:Clinical pharmacists implement individual treatment for patients can improve the clinical efficacy , reduce the risk of medication and fully reflect the value of the clinical pharmacists . |
