OBJECTIVE： To probe into the role of clinical pharmacists in individual medication treatment.METHODS： Retrospective analysis was conducted on the participation of clinical pharmacists in individual treatment of one patient with renal insufficiency after heart valve replacement. RESULTS： Clinical pharmacists implemented individualized anti-infection and anticoagulant therapy and pharmaceutical care for the patients by the application of individualized drug delivery technology in terms of therapeutic drug monitoring and gene detection,and synthetically consideration of the special pathological state of the patients and drug combination,which improved the clinical efficacy and promoted the rational drug use. CONCLUSIONS： Clinical pharmacists implement individual treatment for patients can improve the clinical efficacy,reduce the risk of medication and fully reflect the value of the clinical
