对肝素诱导血小板减少症替代抗凝治疗的病例分析
目的：肝素诱导的血小板减少症（HIT）是肝素引起的严重的免疫性不良反应。通过临床药师参与治疗一例严重腹腔感染危重患者并发HIT的病例,旨在为临床药师参与HIT治疗提供参考。方法：临床药师根据专业知识,积极参阅相关文献资料,并结合患者的病情,协助临床医师鉴定HIT,制定个体化的替代抗凝治疗方案,并对患者进行药学服务。结果：经过近两周的替代抗凝治疗,患者血小板回升至基础值,调整为口服华法林治疗。结论：临床药师运用自身专业知识,参与临床一线诊治,提供优质药学服务是药学服务转型的一种行之有效的方式。...
