七氟醚和氯胺酮用于小儿扁桃体摘除术麻醉诱导效果差异的研究

Summary:目的探讨七氟醚和氯胺酮在小儿扁桃体摘除术麻醉诱导中的效果差异,为临床选择合适的麻醉药物提供现实依据。方法选择2012年8月至2014年8月于该院实施扁桃体摘除术的70例患儿作为研究对象,随机分为观察组和对照组,每组35例。观察组采用七氟醚进行麻醉诱导,对照组采用氯胺酮麻醉诱导。比较两组患儿的麻醉效果,包括麻醉诱导时间、疼痛视觉模拟评分(VAS评分)、术中摇头和肢动发生率、平均手术时间、苏醒时间、术后拔管时间、气管痉挛和脉搏血氧饱和度(Sp O2)不稳定等并发症发生率。结果与对照组比较,观察组患儿麻醉诱导时间[(1.50±0.30)min]、VAS评分[(1.6±0.4)分]、术中摇头和肢动发生率(5.71%,2/35)均明显低于对照组[(5.12±1.32)min、(2.1±0.9)分、37.14(13/35)],手术时间[(30.8±10.9)min]、麻醉苏醒时间[(5.6±7.1)min]及拔管时间[(5.8±6.4)min]明显短于对照组[(39.2±12.0)、(21.9±5.3)、(22.7±4.9)min],支气管痉挛发生率(8.57%,3/35)及Sp O2不稳定率(0.00%,0/35)显著低于对照组[34.29%(12/35)、25.71%(9/35)],差异均有统计学意义(P〈0.05)。结论七氟醚和氯胺酮在小儿扁桃体摘除术中的麻醉诱导效果均能达到手术要求,但七氟醚在麻醉效果及并发症发生率等方面更具优势,起效快且刺激小,安全可靠,在小儿手术中的应用前景更为广泛。
Bibliography:Objective To study the difference of anesthetic induction effect between sevoflurane and ketamine in pediatric tonsillectomy to provide realistic evidence for choosing suitable narcotic drug in clinic. Methods 70 children patients with tonsillectomy in our hospital from August 2012 to August 2014 were selected as the research subjects and divided into observation group and the control group randomly and equally,35 cases in each group. The observation group was given sevoflurane for conducting anesthetic induction, while the control group adopted ketamine for conducting anesthetic induction.
