Summary:To analyze incident reports from orthoptists in outpatient clinic of Inouye Eye Hospital. The analysis included years of experience, date and time to occur, and pressure of business at the incident. Most incidents occurred during instillation of eye drop or ophthalmological testing. Though there were no reports of serious accidents, a lot of incidents may potentially be serious such as instillation of wrong eye drop or for wrong patient. There were a lot of incidents including errors concerning eye drops or guidance by less-experienced orthoptists. While there were many incidents concerning descriptive error in prescription of glasses by more- experienced orthoptists. These incidents results from confirmation shortage, frequency of the incidents depended upon not only years of experience but also types of work. More incidents occurred at relatively busy time. However, the reports described the time as under usual pressure of business rather than under high pressure. It seemed that the reporters did not interpret the incident as consequence of individual status of business. This suggests the incidents may occur inadequate systemic procedures in command or confirmation. Therefore, it was thought that the incidents such as “Confirmation shortage” “Change in the work contents by years of experience”, and “High pressure of business” had been generated.
