| Summary: | Gastrointestinal carcinoid is commonly found in the duodenum. We experienced two cases of carcinoid tumor of the duodenum; one case of advanced carcinoid tumor of the Vater's papilla and another case of early carcinoid tumor of the second portion of the duodenum. Patient 1, a 53-year-old woman was admitted to the hospital for epigastralgia. We detected a protruded tumor 20mm in diameter at the papilla, and a pancreatoduodenectomy was performed. Patient 2, a 57-year-old woman, was pointed out having an abnormality of the stomach at a health screening. We detected an elevated tumor 10mm in diameter at the 2nd portion endoscopically. A partial resection of the duodenum and a right nephrectomy were concurrently performed because a right renal tumor was also detected. Since early carcinoids seem to be submucosal tumor in morphology, constructive biopsy is useful for making the diagnosis. A local resection including endoscopical resection may be indicated for the tumor if it is under 10mm in diameter or its histologic invasion is limitted to the layer of musclar propria. But radical operation is necessary in accordance with carcinoma, if the size of tumor is over 20mm in dameter or histologic invasion is above the layer of musclar propria. |
