Summary:Spigelian hernia is extremely rare among primary abdominal wall hernias. This report describes a case of Spigelian hernia in a 78-year-old woman, with a review of the literature. At the level of 4cm below the umbilicus, a soft bulge was observed by the lateral border of rectus abdominis muscle. By abdominal computed tomography, the aponeurosis according with the region of the bulge was defected. A Spigelian hernia was diagnosed and the patient was operated on. The hernia sac was found under the aponeurosis of external oblique muscle and reduced into the abdominal cavity. The sac and hernia opening were closed by nonabsorbable suture and the fragile portion was reinforced with a Marlex® mesh. Spigelian hernia is an abdominal hernia occurs between the semilunar line and the lateral border of rectus, and only 15 cases have been reported in the domestic literature. For the diagnosis of this disease, a confirmation of the presence of the aponeurotic defect using computed tomography is useful. For the method of treatment, a closure of the hernia opening with prosthetic meshes is available.
