| Summary: | Objective To systematically evaluate the efficacy and safety of moxibustion in treating constipation after stroke, and to provide better evidence-based medical evidence for clinic. Methods Randomized controlled trials of moxibustion in treating constipation after stroke in CNKI, VIP, Wanfang Data, CBM, PubMed, Embase, Web of Science and The Cochrane library were searched by computer. The retrieval time was from the database establishment to June 31, 2024, and the relevant original data of the literature were extracted. RevMan 5. 4 software was used for data analysis, and funnel chart was used for publication bias analysis. Results A total of 14 articles with 953 patients were finally included. Meta-analysis showed that moxibustion significantly improved the total effective rate of constipation treatment compared with the control group (RR=1. 19, 95%CI: 1. 12~1. 27); Significantly reduced constipation symptom score (MD=-2. 33, 95%CI: -2. 90~-1. 77) and Patient Assessment of Constipation-Quality of Life (PAC-QOL) score (MD=-7. 55, 95%CI: -9. 79~-5. 30); Improved Bristol Stool Form Scale (MD=1. 26, 95%CI: 0. 93~1. 58]); Improved Cleveland Constipation Score (CCS) score, and with few adverse reactions. GRADE evidence quality evaluation showed that the total effective rate and constipation symptom score were low evidence quality, Bristol Stool Form Scale was high evidence quality, and PAC-QOL was medium evidence quality. Conclusion Moxibustion can improve the curative effect and clinical symptoms of constipation after stroke, but it needs more high-quality literature to verify it. (目的 系统评价灸法治疗中风后便秘的疗效和安全性, 为临床提供更好的循证医学依据。方法 计算机检索中国知网(CNKI)、维普(VIP)、万方(Wanfang Data)、中国生物医学文献数据库(CBM)、PubMed、Embase、Web of Science、The Cochrane library数据库中有关灸法治疗中风后便秘的随机对照试验, 检索时限为从数据库建库至 2024年6月31日, 提取文献的相关原始数据, 采用 RevMan 5. 4 软件进行数据分析, 并采用漏斗图进行发表偏倚分析。结果 最终纳入14篇文献, 共计953例患者。Meta分析结果表明, 与对照组相比, 灸法提高5便秘治疗总有效率(RR=1. 19, 95%CI: 1. 12~1. 27); 降低便秘症状积分(MD=-2. 33, 95%CI: -2. 90~-1. 77)和便秘患者生活质量量表(PAC-QOL)评分(MD=-7. 55, 95%CI: -9. 79~-5. 30); 提高Bristol大便性状评分(MD=1. 26, 95%CI: 0. 93~1. 58); 改善便秘患者临床评分量表(CCS)评分且不良反应少。GRADE 证据质量评价显示, 总有效率和便秘症状积分为低级证据质量, Bristol大便性状评分为高级证据质量, PAC-QO评分L为中等证据质量。结论 灸法可改善中风后便秘患者的疗效和临床症状, 但需更多高质量的文献加以验证。) |
