외상에 의해 발생한 내직근과 하직근파열 및 시신경 절단 1예

목적: 외상 후 발생한 내직근과 하직근 파열과 함께 동반된 시신경 절단 환자에서 수술적 치료를 통해 좋은 결과를 얻은 1예를 보고하고자 한다. 증례요약: 39세 남자 환자가 외상으로 인하여 내직근과 하직근의 파열과 함께 시신경 절단이 발생하여 내원하였다. 절단된 내직근과 하직근은 안구 부착부위에 봉합하였으며 절단된 시신경은 찾지 못하였다. 수술 후 6개월 뒤 제일눈위치에서 정위를 보였으며 전안부허혈은 발생하지 않았다. 결론: 외상으로 내직근과 하직근 파열과 함께 드물지만 시신경의 절단이 발생할 수 있으며, 수술적 치료로 좋은 해부학적 결과를 얻을 수 있었다. Purpose: To report the good surgical results of multiple ruptured rectus muscles with avulsion of the optic nerve. Case summary: A 39-year-old male patient underwent surgical exploration after rupture of the inferior and medial rectus muscles and avulsion of the optic nerve. The disinserted muscles were attached at the primary insertion site, and a served optic nerve was not found. Six months after the injury, the patient had orthotropia in the primary position without ischemia of the anterior segment. Conclusions: In rare instances, blunt trauma can result in optic nerve avulsion with ruptured inferior and medial rectus muscles. Surgical treatment can result in a favorable outcome.
