안구 둔상에 의한 안구합병증과 동반된 안와골절에 대한 분석

목적: 안구 둔상에 의한 안구합병증 및 안와골절에 대하여 조사하여 안구합병증의 정도를 예측하는 데 도움이 되고자 한다. 대상과 방법: 고신대학교복음병원을 방문한 안구 둔상 환자 168명 169안을 대상으로 후향적으로 조사하였다. 성별, 나이, 키, 체중, 안구 둔상의 원인, 안구합병증에 대하여 조사하였고 컴퓨터단층촬영을 시행하여 안와골절 유무에 따라 ‘안와골절군’, ‘안와좌상군’으로 나누었다. 안와골절의 위치, 앞뒤길이, 너비, 넓이를 측정하였다. 결과: 안와골절군의 40.2% (27/67)에서 안구합병증이 관찰되었고 안와좌상군에서는 73.5% (75/102)에서 안구합병증이 관찰되어 안와골절군에서 유의하게 빈도가 낮았다(p＜0.001). 안구 둔상의 원인에서는 ‘물체가 날아와 눈에 부딪힌 경우’에서 안구합병증이 85.1% (40/47)로 다른 원인에 비하여 높게 나타났다. 안와골절군에서 안구합병증의 유무에 따라 골절의 위치, 앞뒤길이, 너비, 넓이를 비교하였을 때에는 유의한 차이를 보이지 않았다. 결론: 안구 둔상 환자에서 안와골절이 있을 경우 안와골절이 없는 경우에 비하여 안구합병증의 빈도가 유의하게 낮았고 안구 둔상의 원인 중 ‘물체가 날아와 눈에 부딪힌 경우’에서 안구합병증의 발생률이 높게 나타났다. Purpose: To help predict the severity of ocular complications in orbital blunt trauma by analyzing clinical features of ocular complications and orbital wall fracture. Methods: A retrospective survey consisting of 169 eyes of 168 patients with orbital blunt trauma who visited Kosin University Gospel Hospital was performed. The patients’ age, gender, height, weight, cause of accident, and ocular complications were investigated. The patients were imaged using computer tomography and divided into two groups according to whether they had orbital wall fracture: the orbital contusion group and the orbital wall fracture group. Variables of orbital wall fracture, including the location, length, width, and area were measured using computer tomography in the orbital wall fracture group. Results: The incidence of ocular complication was 27 of 67 eyes (40.2%) in the orbital wall fracture group and 75 of 102 eyes (73.5%) in the orbital contusion group; ocular complications were found significantly more often in the orbital contusion group than in the orbital wall fracture group (p < 0.001). Among causes of the accident, ‘flying object’ showed the highest incidence of ocular complication at 40 of 48 eyes (85.1%). In the orbital wall fracture group, there was no significant difference in orbital fracture location, length, width or area between the ocular complication group and the non-ocular complication group. Conclusions: Ocular complications were found significantly more often in the orbital contusion group than in the orbital wall fracture group, and ‘flying object’ showed the highest incidence of ocular complication among causes of the accident.
