비알코올지방간을 가진 한국 남성에서 ALT 증가와 혈중 adiponectin치의 연관성

목적: 비알코올지방간질환(nonalcoholic fatty liver disease, NAFLD)은 단순 지방증과 지방간염을 포함하며, 간경변증으로 진행할 수 있다. 최근 지방세포에서 분비되는 adiponectin, resistin, leptin 같은 adipokines과 비알코올지방간의 위험 인자로 알려진 비만, 당뇨병과 고지혈증 등 대사증후군 사이에는 밀접한 관련성이 있음이 알려져 있다. 본 연구에서는 비알코올지방간으로 진단된 한국 남성을 대상으로 혈중 adiponectin, resisitin, leptin 농도의 혈청 ALT치 상승에 따른 변화를 알아보고자 하였다. 대상과 방법: 건강검진 수진자 중 음주력, 자가면역성 또는 대사성 간질환 및 바이러스 간염 병력 없이 간초음파상 지방간 소견을 보인 38명의 남성을 대상으로 연령, 신장, 체중, 체질량지수 및 간기능검사, 지질검사, 혈당, 인슐린 그리고 adiponectin, resistin, leptin을 측정하였고, 인슐린 저항성을 알아보기 위해 HOMA-IR을 계산하였다. 대상 환자를 ALT치 정상군(40 IU/L 이하, n=28)과 ALT치 상승군(41 IU/L 이상, n=10)으로 나누었다. 두 군간의 비교에서, adiponectin과 resistin에 대해서는 독립 T-검정으로, leptin에 대해서는 Mann-Whitney U-검정으로 비교하였다. 또한 ALT치와 adiponectin, AST 그리고 γ-GT에 대한 상관관계를 보기 위해 다중 회귀분석을 하였다. 결과: 양군 간에 연령, 신체계측 지수, 혈압, 지질검사, HOMA-IR 그리고 Quicki는 유의한 차이를 보이지 않았다. 혈중 adiponectin 농도는 ALT치 상승군에서 ALT치 정상군보다 유의하게 낮았으나(3.89±1.77 vs 7.01±2.54 µg/mL, P=0.001), leptin (4.02±2.04 vs 3.26±1.41 ng/mL, P=0.245)과 resistin (80.14±14.8 vs 80.5±11.34 ng/mL, P=0.937)은 두 군 간에 유의한 차이가 없었다. 또한 ALT와 adiponectin은 음의 상관관계(regression coefficient=-1.318, P=0.038)를 보였다. 결론: 비알코올지방간 환자에서 ALT치 상승에 따라 혈중 adiponectin 농도는 감소하였으며, resistin과 leptin은 유의한 변화가 없었다. 따라서 혈중 adiponectin 농도 감소는 비알코올지방간에서 ALT의 상승과 연관이 있을 것으로 생각되며 향후 adiponectin의 혈중 농도 변화가 ALT치가 상승한 비알코올지방간의 병인에 어떠한 역할을 하는지를 밝히기 위한 연구가 필요하리라 생각된다. Background/Aims: Nonalcoholic fatty liver disease (NAFLD) comprises a large part of chronic liver diseases. Recently it was reported that adipokines are closely associated with the common risk factors for NAFLD, such as obesity, diabetes, dyslipidemia, and insulin resistance. We aimed to evaluate the changes in serum adiponectin, resistin and leptin concentrations related to alanine aminotransferase (ALT) elevations in Korean men with NAFLD. Methods: We studies 38 men who were diagnosed with fatty liver by abdominal ultrasonography. None had a history of excessive alcohol consumption, autoimmune hepatitis, inherited or metabolic liver disease or viral hepatitis. The subjects were divided into two groups. One group had normal levels of ALT (n=28) and the other had increased ALT (n=10). We compared anthropometrical parameters, biochemical items and serum adipokine levels between these two groups. Results: Serum adiponectin levels were lower in the increased ALT group than in the normal ALT group (3.89±1.77 vs 7.01±2.54 µg/dL, P=0.001). But there were no significant differences in serum leptin and resistin levels between two groups (4.02±2.04 vs 3.26±1.41 ng/mL, p=0.245, 80.14±14.8 vs 80.5±11.34 ng/mL, P=0.937, respectively). Multiple linear regression analyses demonstrated that the serum adiponectin level is inversely correlated with serum ALT level and that the serum aspartate aminotransferase (AST) level is positively correlated with the serum ALT level. Conclusions: Our study shows that hypoadiponectinemia is associated with an ALT elevation in patients with NAFLD. Adiponectin may play an indirect role in the development of NAFLD.
