수정체유화술과 연성인공수정체 삽입술 후 절개창의 크기 변화

백내장 적출시 각막절개도에 의해 최초로 형성된 절개창의 크기가 수술 종료시까지 그대로 유지된다는 가설과 달리 수술 각 단계마다 절개창의 크기가 변한다는 연구결과가 제기되어 비교연구를 시도하였다. 100안을 대상으로 특수하계 고안된 계측기를 이용하여 최초 절개창 형성 후 및 수정체유화술 후, 절개창 확대 후, 연성인공수정체 삼입 후에 각각 수술창의 크기를 측정하였다. 50안씩 나누어서 절개창 확대를 3.Omm로 시행한 군을 A, 3.2mm로 시행한 군을 B로 정하였다. 절개창의 변화는 A군은 수정체 유화술 후 0. 06mm, 연성인공수정체 깝입 후 0. 19mm, B군은 수정체유화술 후 0. 05mm, 연성인공수정체 삼입 후 0. 1mm의 통계적으로 유의한 절개창의 확대를 보였다. 소절개에 의한 백내장 수술기술과 수술 후의 회복에 대한 향후의 연구는 최초 절개창의 크기가 최종 절개부위의 크기를 결정할 것이라는 잘못된 오류가 없도록 절개부위의 실질적인 치수를 측정할 것을 강조하는 바이다(한안지 39:1165-1169, 1998). As a study that the incision size was changed at each step of the surgical procedure but it is different, from the universal assumption that. the incision width initially created by a keratome during cataract extraction remained same size after the end of the surgical procedure, the authors investigated a comparative study between the two studies. We used specially designed instrument for accurate measuring of the incision size in 100 eyes and the incision size was measured after the creation of the initial incision, after the coirlpletion of phacoemulsification, after widening the incision and after the folded IOL insertion. Dividing group A and B in 50 eyes respectively, we designated the incision enlargement with 3.0mm keratome as A and 3.2mm keratome as B. It appeared statistically significant widening of the incision by approximately 0.06mm after the completion of phacoemulsification, 0.19mm after the folded IOL insertion in group A and 0. 05mm, 0.1mm in group B, respectively. Future studies of the surgical techniques and the postoperative healing after small incision cataract surgery must measure the actual incision size to avoid misleading assumption that the keratome size at the beginning of the procedure determines the incision size at. the end of the procedure (J Korean Ophthalnaol Soc 39:1165-1169, 1998).
