다공성 안와충전물 삽입술 후 발생한 칸디다 감염 1예

목적 : 안와내 MedporⓇ 삽입술을 시행한 후 발생한 칸디다 진균 감염에 대해 보고하고자 한다. 대상과 방법 : 약 3년전 안와내 MedporⓇ 삽입술을 시행받은 50세 남자가 통증, 안검의 부종, 분비물을 주소로 내원하여, 검사상 충전물의 노출과 감염소견을 보였다. MedporⓇ 제거술을 시행하였으며, 혈액한천배지, potato dextrose 한천배지, CHROMagar 한천배지, 그리고 자동동정화된 효모표준검사에서 Candida parapsilosis가 검출되었다. 결과 : 수술 6개월째 염증없이 창상이 잘 치유되었으며, 경도의 상측구 변형을 제외하고 의안착용시 미용상 만족할 만한 상태를 보였다. 결론 : MedporⓇ 안와 충전물 삽입술 후 통증, 부종 등의 염증소견과 함께 충전물의 노출이 장기간 진행할 경우, 드물지만 진균을 포함한 충전물의 감염을 고려해야 할 것으로 생각된다. Purpose: To report a case of candida infection after MedporⓇ orbital implantation. Methods: A 51-year-old man was admitted complaining of painful swelling and discharge about 3 years after orbital MedporⓇ implantation. External examination revealed signs of infection and exposure of the implant. MedporⓇ was surgically removed. In laboratory studies, white colony was detected in blood agar, potato dextrose agar, and CHROMagar. Automatic identification with yeast standard test revealed Candida parapsilosis. Results: Six months after surgery, the wound had healed well. In addition, cosmesis was acceptable with the artificial eye, except mild superior sulcus deformity. Conclusions: Although rare, fungal infection of the implant should be suspected when there are the long standing signs of infection and exposure after orbital MedporⓇ implantation.
