開放性ウエットドレッシング法による褥創の治療経験

` Shunichi TORIYABE M.D. et al. established the Open Wet-dressing Therapy' (OpenWT) for the treatment of pressure ulcers. Characteristics of applied OpenWT were 1. simple manual needed less skills, 2. Nurse could make shortened the care 3. performed cost down.. We introduced the OpenWT therapy from April 2006. The method we performed, 1. Wash decubitus by a sprayer. 2. Absorb water with gauze. 3. Wrap decubitus using the opsite flex. 4. Repeat procedure 1-3 everyday and event of hazuddus. 5. during the passage We evaluated decubitus. At the results, Case 1, 2 and 3 the ulcers healed after 65 days, 61 days and 112 days respectively. 開放性ウエットドレッシング療法 (OpenWT) は鳥谷部俊一らが提唱する褥創の治療法である。 OpenWTは手技が簡単でコストも安く処置時間も短くなるという利点がある。当院ではH18年 4月より徐々にOpenWTを取り入れ褥創患者に実践している。方法は褥創を霧吹きにて水道水で洗浄し創周囲の水分をガーゼでふき取りオプサイトフレックスで覆った。原則として一日一回の処置で汚染時は適時交換とした。定期的に創部の評価を行い良好な結果を得られた。また創傷治癒遷延の原因となる貧血・低栄養状態の改善を期待して漢方薬を併用した。改善を認めた症例の分析結果を比較報告する。これ等の症例報告より一ヶ月程度の教育入院にて家族にこの処置を理解してもらえれば、在宅でも褥創管理は十分可能と思われた。
