Abstract 老年女性に多くみられる亀背が呼吸機能の加齢変化に及ぼす影響を心肺疾患, 神経筋疾患がなく, FEV1/FVC>0.7で明らかな呼出障害を示さない20歳から94歳までの成人300名 (男性170名 (平均年齢53.9±1.5歳), 女性130名 (平均年齢56.2±1.6歳)) を対象として検討した. 胸部X線上で脊柱後彎の程度を Cobb 角として評価した. スパイロメトリー, 肺気量分画, 呼吸筋力を測定し, これら指標の加齢変化に及ぼす脊柱後彎 (Cobb 角) の影響を重回帰分析で検討した. 努力肺活量 (FVC), 一秒量 (FEV1), 全肺気量 (TLC), 最大吸気圧 (PImax), 最大呼気圧 (PEmax) は経年的に減少し, 残気量 (RV), 残気率 (RV/TLC) は増加した. 脊柱後彎の程度 (Cobb 角) は加齢とともに増加した. Cobb 角はTLC, VC, FEV1, PImax, PEmax と有意の負の相関を認め, RV, RV/TLCと正の相関を示した. さらに, Cobb 角は, PImax, RV/TLC, VCについて年齢とは独立した予測変数であった. したがって, 脊柱後彎の進行は, 吸気筋の発生圧を低下させ, 残気率を増加し, 肺活量を制限して, 加齢による呼吸機能の低下を一層悪化させる可能性が考えられた.
