Subjects and Methods : Computed tomography (CT) images from 46 unilateral neutrophilic rhinosinusitis patients were retrospectively reviewed, and examined for a causal relationship between the incidence of chronic inflammation and NSD. In all patients, there were no mucosal abnormalities at all in the non-diseased sides. We originally defined the NSD subgroup and non-NSD subgroup based on the width of the OMC between the septum and the lateral wall. The Lund-Mackey Staging System was adopted for the evaluation of the volume of inflammation within the paranasal sinuses. The correlation between the NSD and CRS was evaluated using the Mann-Whitney's U-test. Results and Conclusions : A statistically significant association was observed between the staging of the maxillary sinus and NSD (p < 0.05). We consider that NSD causes CRS. We therefore recommend that functional endscopic sinus surgery should be performed in combination with septal plasty. |

鼻中隔彎曲症が慢性副鼻腔炎の遷延化に及ぼす影響について、解剖学的構造異常以外の疫学的要因をかなり無視できると思われる片側性副鼻腔炎 46例を対象に研究を行った。患側凸群 13例・彎曲なし群 15例・患側凹群 18例に分類し、Lund-Mackay Staging System に準じて患側副鼻腔の重症度を評価した。彎曲の方向にかかわらず、中鼻道に排泄路をもつ副鼻腔の重症度が高かった。患側凸群と非患側凸群 (彎曲なし＋患側凹) の比較において、上顎洞炎の重症度に有意差を認めた (p < 0.05)。以上の結果から、上顎洞は慢性副鼻腔炎の好発部位であり、さらに鼻中隔彎曲は上顎洞炎の遷延化因子になることが示唆された。 Introduction: There are pros and cons regarding the pathogenesis of chronic rhinosinusitis (CRS) by nasal septal deviation (NSD) around the ostiomeatal complex (OMC).
