経静脈的塞栓術により脳幹浮腫の改善を認めた海綿静脈洞部硬膜動静脈瘻の1例

Abstract 【目的】進行性に脳幹浮腫を呈した海綿静脈洞部硬膜動静脈瘻(CS-dAVF)の患者に対して,迅速な経静脈的塞栓術により良好な結果を得ることができた1例を経験したので報告する.【症例】80歳女性.結膜充血・複視にてCS-dAVFを発症した.頭部MRI/MRAでは,両側海綿静脈洞外側後方に流入血流を認め,右上眼静脈の著明な拡張を認めた.経過観察中に眼症状が増悪し,MRI再検により,右延髄・右橋・右中小脳脚に新たに浮腫の出現を認めた.脳血管撮影検査にて,海綿静脈洞(CS)からpontine bridging vein(PBV)を介する脳幹部への静脈流出を認めた.左下錐体静脈洞経由でCSに到達し,PBV起始部を含めてコイル塞栓を施行し,完全閉塞が得られた.術直後から眼症状は改善し,脳幹浮腫も改善した.【結論】脳幹浮腫を有するCS-dAVFにおいて,早急な治療介入により症状増悪が予防できる可能性が示唆された.
