S2 alar iliacスクリューの折損ならびにセットスクリューの脱転をきたした3例
症例1:79歳男性.L4椎体骨折後の後側弯変形に対してT7から骨盤までの前後方矯正固定術を施行.術後9ヶ月で右S2 alar iliac(S2AI)スクリューネック部での折損が判明,腸骨スクリューを左右2本ずつ追加し再固定した.症例2:72歳女性.T12椎体骨折後の後弯変形に対してT6から骨盤までの後方矯正固定術を施行.術後2ヶ月で両S2AIスクリューネック部での折損が判明,両側S2AIスクリューの再挿入と右S1よりSacral alar iliac screwスクリューを1本追加し再固定した.症例3:77歳男性.腰椎複数回手術に対してT8から骨盤までの前後方矯正固定術を施行.術後1年半で両側...

Abstract | 症例1:79歳男性.L4椎体骨折後の後側弯変形に対してT7から骨盤までの前後方矯正固定術を施行.術後9ヶ月で右S2 alar iliac(S2AI)スクリューネック部での折損が判明,腸骨スクリューを左右2本ずつ追加し再固定した.症例2:72歳女性.T12椎体骨折後の後弯変形に対してT6から骨盤までの後方矯正固定術を施行.術後2ヶ月で両S2AIスクリューネック部での折損が判明,両側S2AIスクリューの再挿入と右S1よりSacral alar iliac screwスクリューを1本追加し再固定した.症例3:77歳男性.腰椎複数回手術に対してT8から骨盤までの前後方矯正固定術を施行.術後1年半で両側のS2AIスクリューのセットスクリュー脱転が判明,SAIスクリューの再挿入と両S1スクリューを追加し再固定した.結語:脊椎インプラントには,骨癒合が得られるまでインプラント連結部の最脆弱部分に応力が集中する.太径で長いS2AIスクリューでも破損を生じうることを認識する必要がある. |
