Anterior column realignmentにおける局所前弯獲得不良因子の検討

Abstract | はじめに:Anterior column realignment(以下ACR)とextreme lateral interbody fusion(XLIF),経皮的椎弓根スクリュー(以下PPS)を用いた低侵襲前後方固定術におけるACR施行椎間の局所前弯(以下SL)獲得不良要因について調査した.対象と方法:2018年以降ACRとXLIF,後方はPPSを用いて手術施行した成人脊柱変形(ASD)患者74名とした.術後SLが17°以上のG群と術後SL 17°未満のP群に分け,使用cage前弯角及びcage高,術前椎間板性状,ACR施行レベル,固定椎間数,脊柱骨盤パラメーター,SL,前方及び後方椎間板高(ADH,PDH),cage設置位置に関して検討した.術前椎間板性状は変性が少ない前弯(type N)変性が少ない後弯(type K),片側変性(type U),椎間板腔消失(type D),前方変性(type A),後方変性(type P)の6つに分類した.結果:二群間で年齢,性差,使用cage前弯角,cageの高さ,cage設置位置に関して有意差はなく,固定椎体数はG群で有意に多く,術前椎間板性状はP群で有意にtype Aが多かった.各種パラメーターは二群間で有意差はなく術前後SLはP群で有意に小さかった.術前PDHはP群で有意に高かった.多変量解析の結果,術後SL 17°未満となるリスク因子は術前PDH,術前SLが確認された.ROC解析にてカットオフ値は術前SL 4°,術前PDH 3.7 mmであった.結語:ACR施行椎間における術後SL獲得不良因子は術前の小さいSL,及び術前の大きなPDHであった. |
