| Abstract | この後向き研究の目的は, 垂直性骨欠損に対しスケーリング・ルートプレーニング (SRP)のみ, またはSRPとOpen Flap Debridement 併用(SRP+OFD)を行い, その後メインテナンスを行った群と歯周治療もメインテナンスも行わなかった群を比較し, その予後を検討することである。被験者と被験部位は北海道医療大学歯科内科クリニック保存Iに通院している慢性歯周炎患者(歯周治療群)40名の100部位と, 慢性歯周炎と診断されたが歯周治療へ参加せず, 治療を途中で中断しメインテナンスも行わなかったグループ(対照群)10名の36部位であった。初診時のエックス線写真で垂直性骨欠損が認められ, SRPのみまたは, SRP+OFDを行った後, 3年以上経過した部位を対象とした。大臼歯の根分岐部病変に連続する垂直性骨欠損は除外した。検査項目の調査は処置時, 処置3年後, およびメインテナンス中の直近の来院時に行った。年齢, 性別, 喫煙の有無, 治療内容, 歯肉炎指数(GI), プロービング時の出血の有無(BOP(+)), プラークコントロールレコード(PCR)および4mm以上の歯周ポケット深さの割合(PPD>=4mm)を検査した。初診時と治療後3年以上経過したエックス線写真により, セメントエナメル境と骨欠損底部の距離を測定した。 歯周治療群の処置後平均経過年数は10.4±4.7年であった。歯周治療群では処置後骨欠損深さは減少し, その後維持された。対照群の初診後平均経過年数は4.5±1.2年であり, 骨欠損深さは増加した。歯周治療群ではSRPとSRP+OFDでは骨欠損深さの減少量に有意差は認められなかった。骨欠損角度が45°より大きい場合よりも45°以下のほうが骨欠損深さは改善した。処置時にBOP(＋)が認められても歯周ポケットは改善したが, 直近にBOP(＋)があるとメインテナンスを行っても歯周ポケットの悪化が見られることが明らかになった。本研究結果から垂直性骨欠損に対してSRPやOFDを行い, 定期的なメインテナンスを行うことにより予後は良好であるが, 術後の炎症のコントロールが重要であることが示唆された。 日本歯周病学会会誌(日歯周誌)52(2) : 161-169, 2010 |
