Abstract | はじめに:S1椎弓根スクリュー(PS)は解剖学的に力学的に緩みやすく,仙椎前方の皮質骨を貫くbicorticalやtricorticalが提唱されているが,ガイドワイヤーを使用する経皮的椎弓根スクリュー(percutaneous pedicle screw,PPS)では神経血管損傷のリスクを伴う.L5/S椎体間固定術におけるS1 PPSの軌道,血管損傷のリスクについて後ろ向きに検討した.対象と方法:L5/Sを含む2椎間以下の椎体間固定術において76例,S1 PS 152本(男性49例,女性27例,平均年齢68±13歳)を対象とした.S1 PPSの軌道は前方皮質穿破unicorortical fixation(Uni)群,前方皮質穿破ありbicortical fixation(Bi)群,mPES(modified penetrating endplate screw)法で挿入されたS1終板を貫くmPES群に分け,血管損傷リスク(ガイドワイヤー軌道上の血管の有無,PS先端と血管距離≦5 mm)について検討した.結果:症例はmPES群10例,両側Bi群15例,片側Bi群21例,Uni群30例で,S1 PSはmPES群19本,Bi群51本,Uni群82本であった.ガイドワイヤー軌道リスクはmPES群:5.26%,Bi群:58.8%,Uni群:30.5%で有意差を認め(p<0.01),PS先端リスクではmPES群:5.26%,Bi群:52.9%,Uni群:7.32%で有意差を認めた(p<0.01).結語:S1 PPSにおけるbicortical purchaseはスクリュー内側角が小さく血管損傷リスクが高い.mPES法は血管損傷のリスクを低減できS1 PPSでは有用である. |
