「緒言」Le Fort I型骨切り術は, 顎変形症治療において欠かせない手術術式である. Le Fort I型骨切り術により安定した上顎骨の移動が可能であるが, 今日の顎変形症治療においてはその精度と同時に安全な手術が求められている. Le Fort I型骨切り術における合併症は, 垂直方向の離断すなわち翼突上顎縫合離断とダウンフラクチャーの際に起こると報告されている. さらに翼突上顎縫合離断で起こる合併症をBreezeらが報告し, 翼状突起部の予期せぬ骨折は軽度の出血から視神経麻痺など重篤な合併症をも引き起こしうるとしている. この合併症リスクを軽減するべく, 1983年TrimbleらはLow levelでの分割であれば解剖学的に危険な事が少ないとして上顎結節内で離断する方法を提唱した. 翼突上顎縫合離断にオステオトームを使用しない, あるいはオステオトーム使用は上顎結節内に留めるとした報告は, いずれも予期せぬ翼状突起骨折のリスクを下げる工夫として報告されている.
