リンパ行性転移と考えられる前立腺癌直腸転移の1例

症例は53歳の男性で，便秘，腰背部痛を主訴に当院を受診した．下部消化管内視鏡検査で下部直腸に全周性の病変を認めた．同部位の生検組織から低分化腺癌が確認された．また，血清PSA値が高値であり，前立腺生検を施行したところGleason score 9点で，前立腺癌の診断となった．CT，PET-CTより硬化性の転移性骨腫瘍の所見は認めたが，前立腺と直腸の間は比較的明瞭であり，前立腺癌骨転移と原発性直腸癌の併存と診断した．原発性直腸癌に対して腹腔鏡下Hartmann手術を施行した．病理組織検査では上皮性の異型は乏しく，粘膜下から漿膜側を主体に低分化腺癌の浸潤増殖を認め，リンパ管内にも腫瘍細胞の充満を認...
