十二指腸分類不能紡錘腫瘍の1例

症例は45歳，男性．黒色便で近医を受診し，上部消化管内視鏡検査で十二指腸腫瘍を認めた．生検結果からは，SYT遺伝子の転座は認めないが，免疫染色や組織像から十二指腸滑膜肉腫と診断され，亜全胃温存膵頭十二指腸切除術を施行した．しかし，術後の切除検体からもSYT遺伝子転座は認めず，滑膜肉腫は否定された．EWSR1の転座は認めたが，融合遺伝子までは確定できなかった．組織像からは，round cell sarcoma with EWSR1-non-ETS fusionsという疾患概念に当てはまる可能性を指摘されたが，最終的には分類不能の紡錘細胞腫瘍との診断に至った．Round cell sarcoma...
