腹腔鏡と臀溝皮弁を併用して修復した直腸切断術後会陰ヘルニアの1例

症例は66歳の男性で，直腸癌に対し，腹腔鏡下腹会陰式直腸切断術および開腹下左側方リンパ節郭清施行後1年のCTにて，会陰ヘルニアの診断となった．徐々に増大し，術後5年目に会陰部の小児頭大の膨隆と皮膚潰瘍による疼痛でQOLを著しく損なっていたため，根治術を施行した．腹腔鏡下に骨盤底のヘルニア門を覆うようにメッシュを留置し大網で被覆し，会陰操作で内陰部動脈の穿通枝皮弁である臀溝皮弁で補強した．術後1年無再発経過中である．本症例では，経腹的アプローチのみでは術後漿液腫出現や腹腔内臓器の圧に耐えきれずに再発するリスクが懸念され，また経会陰アプローチのみでも修復が不十分となる可能性を考慮し，本術式を選択し...
