脳卒中における血管内治療の現状と新展開

脳卒中疾患の中でも血管内治療の機会が多い,破裂脳動脈瘤のコイル塞栓術,頸動脈狭窄に対するステント留置術,急性期脳梗塞の機械的血栓回収療法について,関連する最近の大規模臨床試験の結果も含め概説する。まずコイル塞栓術では,様々なコイルおよび補助デバイスの登場により治療適応が拡大され,さらには治療成績の向上につながっていることを述べる。ステント留置術については,虚血性合併症の低減のために使用される各種デバイスと今後の課題について述べる。最後に,急性期脳主幹動脈閉塞に対する新たな治療である機械的血栓回収療法及び使用されるMerci RetrieverとPenumbra systemについて解説する。...
