乳腺転移をきたした前立腺癌の1例

前立腺癌の乳腺転移は稀である．今回，乳腺転移をきたした前立腺癌の1例を経験したので報告する．患者は85歳，男性．1997年3月，前立腺癌（T3N0M1，stage D2）と診断され，以降内分泌治療を施行してした．2007年11月，有痛性の左乳腺腫瘤を主訴に当院を受診した．触診，マンモグラフィー，超音波検査などで左乳房C領域に約1cmの腫瘍を認めた．経皮的針生検でscirrhous carcinomaと診断されたため乳房部分切除術を施行した．術後病理組織診断で前立腺特異抗原（PSA）染色及び前立腺性酸性ホスファターゼ（PAP）染色が陽性であり前立腺癌乳腺転移と診断した．画像診断や穿刺吸引細胞診で...

Abstract 前立腺癌の乳腺転移は稀である．今回，乳腺転移をきたした前立腺癌の1例を経験したので報告する．患者は85歳，男性．1997年3月，前立腺癌（T3N0M1，stage D2）と診断され，以降内分泌治療を施行してした．2007年11月，有痛性の左乳腺腫瘤を主訴に当院を受診した．触診，マンモグラフィー，超音波検査などで左乳房C領域に約1cmの腫瘍を認めた．経皮的針生検でscirrhous carcinomaと診断されたため乳房部分切除術を施行した．術後病理組織診断で前立腺特異抗原（PSA）染色及び前立腺性酸性ホスファターゼ（PAP）染色が陽性であり前立腺癌乳腺転移と診断した．画像診断や穿刺吸引細胞診では前立腺癌乳腺転移の診断は難しい場合があり，鑑別診断には免疫染色が有用と思われた．また前立腺癌乳腺転移は終末期患者にみられる予後不良の病態であり，切除の意義は乏しいと思われた．
