開発途上国滞在者のウイルス性肝炎に関する疫学的研究と予防対策の評価

開発途上国滞在者の間に急性ウイルス性肝炎罹患者が高率に発生していることが知られている. 開発途上国滞在者の健康管理に役立てるため, 邦人集団 (年齢20～35歳, 2年間滞在) を対象に急性ウイルス性肝炎の疫学的調査 (調査期間10年) を実施し, 予防対策の効果について検討した. 免疫血清グロブリンの接種を開始する以前はA型肝炎が全体の79%を占めていたが, 接種開始後著しく減少し, 有効性が再確認された. Abstract 開発途上国滞在者の間に急性ウイルス性肝炎罹患者が高率に発生していることが知られている. 開発途上国滞在者の健康管理に役立てるため, 邦人集団 (年齢20～35歳, 2年間滞在) を対象に急性ウイルス性肝炎の疫学的調査 (調査期間10年) を実施し, 予防対策の効果について検討した. 免疫血清グロブリンの接種を開始する以前はA型肝炎が全体の79%を占めていたが, 接種開始後著しく減少し, 有効性が再確認された. B型肝炎と診断された症例35例のうち2例はHBe抗原陽性キャリアからの発症であったが, 他の例は開発途上国滞在中に感染し, 発症した例であった. B型肝炎ワクチンを3回接種後抗体が陽性になった老345人中B型肝炎の発症例は皆無で, ワクチンの接種の有無と発症率に関し有意差が認められた. 滞在中HBs抗原または抗体が陽転する者の割合は1988年では4.5%であり, 東南アジアおよびアフリカ滞在者が特に高い陽転率を示していた. 非A非B型肝炎と考えられる例は10例認められた. 感染経路として経口感染と思われる例が多い. 日本人が開発途上国に滞在する際, 急性ウイルス性肝炎に罹患する危険性が大きい. 感染経路として, 経口感染, 性行為感染, 医療行為によるものが重要である. 予防対策として, 免疫血清グロブリンの接種やB型肝炎ワクチンの接種がきわめて有効である.
