症例は52歳，女性．胃潰瘍で近医通院中に心窩部痛・背部痛・食欲低下があり，腹部USで膵腫瘍を指摘され紹介となった．CTでは膵全体に14cm大の濃度不均一な多血性，分葉状の腫瘍を認めた．腫瘍は脾門部と左側大網の静脈内に腫瘍栓を形成し，上腸間膜静脈への浸潤も疑われた．また左腎静脈を圧排，左副腎への浸潤が疑われた．膵神経内分泌細胞腫瘍を疑い，リンパ節郭清を伴う膵全摘，門脈合併切除再建術を施行した．術中所見で左副腎および左腎静脈への浸潤が疑われ，左副腎と左腎を合併切除した．切除標本の病理組織所見では，異型細胞が索状・胞巣状，一部で腺房状構造を呈して密に増殖していた．免疫染色および電顕所見も膵腺房細胞由...

Abstract 症例は52歳，女性．胃潰瘍で近医通院中に心窩部痛・背部痛・食欲低下があり，腹部USで膵腫瘍を指摘され紹介となった．CTでは膵全体に14cm大の濃度不均一な多血性，分葉状の腫瘍を認めた．腫瘍は脾門部と左側大網の静脈内に腫瘍栓を形成し，上腸間膜静脈への浸潤も疑われた．また左腎静脈を圧排，左副腎への浸潤が疑われた．膵神経内分泌細胞腫瘍を疑い，リンパ節郭清を伴う膵全摘，門脈合併切除再建術を施行した．術中所見で左副腎および左腎静脈への浸潤が疑われ，左副腎と左腎を合併切除した．切除標本の病理組織所見では，異型細胞が索状・胞巣状，一部で腺房状構造を呈して密に増殖していた．免疫染色および電顕所見も膵腺房細胞由来を示し，膵腺房細胞癌と診断した．術後は血糖コントロールも含めて概ね良好に経過し，術後46日目に退院となった．術後14カ月経過し無再発生存中であるが，今後も再発に対する厳重な経過観察が必要である．
