オープンステントグラフト治療における末梢灌流の有用性

Abstract | [目的]オープンステントグラフト(OSG)における末梢灌流(DP)の有効性を検討した.[対象と方法]DP施行有無によってNon-DP群(23例),DP群(27例)に分類した.両群間において従前因子に統計学的有意差は認められなかった.[結果]手術時間は両群間に統計学的有意差はなかったが,人工心肺時間(178±22分 vs. 193±18分,p<0.01),大動脈遮断時間(84±23分 vs. 106±19分,p<0.01)はDP群で有意に長く,腹部主要臓器循環停止時間(46±11分 vs. 20±5分,p<0.001)はNon-DP群で有意に長かった.術後初期成績は,Non-DP群で対麻痺および不全対麻痺を各1例認めたのに比し,DP群では永久的脊髄虚血は認められなかった.術後気管内挿管時間(72.6±40.1時間 vs. 40.1±34.7時間,p<0.05)は統計学的有意にNon-DP群で長かった.病院死亡はNon-DP群でStrokeおよび呼吸不全各1例,DP群で呼吸不全1例であった.術後最高値BUN(38.5±15.6 mg/dl vs. 30.8±9.8 mg/dl,p<0.05),術後最高値s-Cr(1.9±1.0 vs. 1.3±0.4 mg/dl,p<0.01)と統計学有意にNon-DP群において高値であった.[結論]OSGにおけるDPは有効な併用法であることが示唆された. |
