IgG4 関連心腫瘍を併発した慢性解離性大動脈瘤の1例
IgG4関連疾患は多臓器に発生する線維硬化性病変であり心血管領域での報告も散見される．今回IgG4関連心腫瘍を併発した解離性上行大動脈瘤に対する手術を経験したため報告する．症例は72歳女性で嚥下時のつかえ感，体重減少を主訴に来院した．心エコーで左室に接した心嚢内腫瘍を指摘され，造影CTで最大短径60 mmの解離性上行大動脈瘤を認めた．胸痛等の解離発症のエピソードはなく画像上も慢性解離の所見であった．腫瘍組織診断のために左開胸下に腫瘍生検を施行した．病理組織学的診断では悪性所見を認めず，IgG4陽性形質細胞の浸潤を認めた．血中IgG4は基準値を超え，他臓器でIgG4関連疾患の所見はなかった．冠動...
